Please be aware
that as of March 1, 2003 paperwork for any and all Florida PAL events
needs to be submitted using the following guidelines or it will be
returned to your PAL.
All youth
participating in Florida PAL events must have the following submitted by
the deadline for each event:
- Participant
Application [only accepting the one revised
March 25, 2002- please do not send the one dated revised Feb 1, 2001
it will no longer be accepted]
- A
Recent PHOTO [FL
PAL will no longer accept copies – they are extremely hard to
lighten] Make sure to tape or clip the photo to the participant
application. You
may submit photos on line in
Jpeg, tiff, gif, or bitmap format.
- Release
of Liability and Indemnity
- The parent and the participating youth must sign
(available online with a special link - contact SFAPAL)
- Emergency
Medical Consent Form [
this
is supposed to be kept by your PAL for your records unless it is for
the Youth Directors Program or something concerning the PAL YEP
GRANT].
- Proof
of Birth
[hospital
forms may not be submitted; if the parent does not have a good
certificate that you can copy please retain a copy of the school
record from local school resource officer]
All adults
participating in Florida PAL events must have the following submitted by
the deadline for each event:
- Participant
Application
- Release
of Liability and Indemnity
All PALs must
submit a T-shirt Breakdown in order to receive T-shirts for FL PAL events {this
form can be found in your rule book or online}
Proper Submission for FL PAL Events
All of your
paperwork should be complete when it is submitted to Florida PAL office.
If any paperwork is missing for the participant he or she will be
dropped.
If paperwork is missing Florida PAL office staff will not be able
to fax you concerning all the missing items due to the turn around on our
events; this is just a luxury we can no longer afford so please make sure
everything is ready before mailing your packet.
FL PAL staff would
appreciate your calling ahead of time if you run into trouble or have
questions and concerns. Staff members cannot assist you to the best of
their abilities if you call after the deadline or if things are missing
after the deadline has past.
We
recommend the use of a check off sheet to make sure that all required
paperwork is in for each participant and for every event.
For
each participant youth and/or adult: paperclip his or her information
together or fold the page back. No staples please. Do not attach the photo to anything but the participant
application.
If
the event requires a roster the paperwork should be lined up (in order
with) with the roster for easier processing.
All
entry fees (if applicable) should be in packet.
FL
PAL will no longer accept overnight packages to the P.O.
Box. If you must overnight a
package to Florida PAL to make the deadline we strongly suggest your PAL utilize Federal Express, UPS, or Airborne etc and overnight
the package to our street address only.
The
United States Post Office cannot guarantee the delivery of your packages
on time. We have had several problems with the post office of late and if
you so decide to use them please be aware that Florida PAL is not
responsible for said choice of overnight service.
Helpful
Hints:
If you have a UPS
account, did you know that in the state of Florida all ground packages are
guaranteed next day but only in the state of Florida? For under
$4.00!
Also every city has a
main Fed Ex, UPS, Airborne, and (your cities main) post office drop off
site that stays open later than 5pm; some even have 8:30pm drop offs and
airport locations with later drop off times, that way you do not have to
rely on a drop box or a pick up from the carrier. Also locate the
mailboxes etc in your communities or businesses like that. They prove
highly useful in a jam. J
Florida
PAL has established these guidelines so that we can provide quality
events. We cannot assure successful events if we cannot be professional
with the companies we have to maintain relationships with. When
paperwork is late, in disarray, and incomplete your state office cannot
provide printers, trophy shops, T-shirt designers, hotels, and catering
staff with the appropriate numbers. Therefore leaving room for mistakes on
crucial items that each PAL participant counts on being right. Please
assist us in our efforts to have a first class organization that sets the
tone for others to follow. Deadlines
are established so that we can have adequate turn around time to do all
the things that help us create a great event for our members. Please help
us so that we can provide professional statewide events for all the
deserving youth we serve. We
