Requirements (2010)
***Each PAL must submit the following on each youth in their Youth
Directors Council***
{on or before September 23, 2010}:

Participant Application
·
Release of Liability
[dated after January 15, 2010]
·
Proof of Birth
·
Color Photos
·
Sign
in at all SFAPAL Events attended
·
Code
of Conduct
EACH PAL must submit the following:
·
Youth Directors Council Attendance/Verification Forms
·
Commitment to PARTICIPATE in the 2010 TAG Campaign
·
(each PAL must have a minimum of 50 Tag commitments submitted to the
state office on or before August 19, 2010)
Each youth director must complete and have documented at State PAL
